    Employment Elective Module: Certificate of Completion Application Form

    Important Notes:

    1. Eligibility: Applicants must have completed at least 20 credits from the module courses.
    2. Fill out the Employment Module Certificate of Completion Application Form and verify the accuracy of the information provided.
    3. Attach the original transcript covering four years of coursework along with the application form, and submit them to the Fashion Design Department Office.
    4. The review process takes 7 working days to complete.

    2023 International Fashion Design Workshop (Japan): Program and Registration Form

    Please download the registration form from the Fashion Design Department website before 12:00 am on Wednesday, March 1st Complete the form and submit it to the department office to Ms. Lai along with a deposit of NT$5,000.
    (Registration is capped at 25 applicants. Once the limit is reached, registration will close early. The department will then select 16 participants, and a pre-departure briefing will be announced for the confirmed participants.)
